Is warm-up in angina ischaemic preconditioning?

are then able to continue exertion with few or no symptoms but, unlike warm-up angina, without an intervening period of rest. Some of these phenomena have been recognised for centuries2 but they now receive less attention because of effective anti-angina treatment and changes in investigative techniques. For example treadmill exercise testing does not allow rest and imposes incremental increases in effort which make the walk through effect less likely. Only about 1 in 5 patients report symptoms of warm-up angina.' Objective testing in recent studies showed that at least 75% of unselected patients with angina had an enhanced performance on second effort.47 In these studies exercise performance on second effort was on average 125% of that on first effort, but there was considerable individual variability. Not surprisingly those individuals who derived the greatest benefit from first effort were those with a history of warm-up

Warm up phenomenon and preconditioning in clinical practice.
The warm up phenomenon, first described more that 50 years ago in patients with effort angina, refers to the improved performance exhibited by more than half of all patients with coronary artery disease following their first exercise test. There are several potential causes of the warm up phenomenon including: (1) an improvement in oxygen supply, which, in turn, may be caused by stenosis dilati...
متن کاملCARDIOVASCULAR MEDICINE Does a selective adenosine A1 receptor agonist protect against exercise induced ischaemia in patients with coronary artery disease?
Background: The “warm up” effect in angina may represent ischaemic preconditioning, which is mediated by adenosine A1 receptors in most models. Objective: To investigate the effect of a selective A1 agonist, GR79236 (GlaxoSmithKline), on exercise induced angina and ischaemic left ventricular dysfunction in patients with coronary artery disease. Design: A double blind crossover study. Patients: ...
متن کاملWarm up angina
Walk-through angina dates back more than 200 years. It was first described in a letter to the British physician, William Heberden. The term is used to describe patients with ischemic heart disease who have reduced symptoms after an episode of angina. It is related to ‘‘warm up’’ angina or, when playing golf, ‘‘first hole’’ angina. The mechanisms responsible for warm up angina are not yet clear,...
متن کاملEffects of K(ATP) channel blockade by glibenclamide on the warm-up phenomenon.
AIMS The increased tolerance to myocardial ischaemia observed during the second of two sequential exercise tests, i.e. the warm-up phenomenon, has been proposed as a clinical model of ischaemic preconditioning. As ATP-sensitive K+ channels appear to be a mediator of ischaemic preconditioning in both experimental and clinical studies, the aim of this study was to investigate the role of K(ATP) c...
متن کاملIschemic preconditioning in humans: models, mediators, and clinical relevance.
Ischemic preconditioning, a powerful form of endogenous protection against myocardial infarction, has been demonstrated in several animal species and, recently, in isolated human cardiomyocytes. For both logistic and ethical reasons, no clinical study can meet the strict conditions of experimental studies on preconditioning with infarct size as the end-point. Nevertheless, the demonstration of ...
